
Washtenaw Health Plan Renewals were sent out on March 13th. It may take a few days or a couple of weeks to arrive.
When you receive the form, please complete it, include a copy of your proof of income, and return or send it to the office before May 31st.
If you do not receive your form, you will have to wait until May 1st to request a renewal at the office and you will need to provide proof of income and address.
If you do not renew by May 31, your WHP coverage will end.
Renewing is simple and only takes 4 steps.

Renew your coverage in 4 Steps:
1. Check your mail

• Forms mailed starting March 13, 2026
• Look for a letter with the Washtenaw Health Plan logo.

2. Complete your form

• The WHP renewal form is one page.
• Use one form per person renewing coverage.
• Check that your name, contact information, and Member ID number are correct.
• Sign the bottom of your form.
3. Add proof of income (REQUIRED)
Include proof of recent income for anyone in your household who works or earns money.
Examples include:
• Recent pay stubs
• Your most recent tax return
• A signed letter from your employer if you are paid in cash
• Copies of checks or payment records
4. Return your form by May 31

Common Questions
I didn’t receive my form.
If you do not receive your form, you will have to wait until May 1st to request a renewal at our office. You will need to provide proof of household income and address.
I moved recently.
If you moved and did not tell us your new address, please come to our office for help.
Bring proof of your new address, such as:
• a utility bill
• a tax letter
• other official mail with your name and address
We must be able to reach you by mail. If we cannot reach you by mail, your coverage will end.
What if I no longer qualify for WHP?
We will help you enroll in other coverage and tell you about low-cost health care options.
How will I know if my coverage was renewed?
We do not send renewal confirmation letters. We will contact you only if there is a problem. If you returned your form and did not hear from us, assume your coverage was renewed.
Do I get a new WHP card every year?
No. If you renew your coverage, you will keep using the same WHP card.
What if I don’t renew?
If you do not renew by May 31, your WHP coverage will end. You will need to re-enroll.
